Individual Tax Return Checklist
Income
- Salary/Wages: Include your Group Certificates/PAYG Payment summaries
- Centrelink Payments Summary (PAYG Forms)
- Eligible Termination Payment: Attach ETP statements for funds withdrawn from Super or Termination payments
- Interest Income: Include all interest received. Please provide bank statements or a summary
- Dividends: Please provide statements of dividends received in the current financial year.
- Partnership/Trust Distributions: Please provide Annual Tax Summaries of trusts (e.g BT, Colonial, AXA)
- Capital Gains on Shares: Include Purchase and Sale details including date, number of shares purchased or sold and the dollar amount paid and received.
- Capital Gains on Properties: Include details of purchase and sale (see Capital Gains on Properties in the Information Section)
- Rental Income: Provide Details of rental income and expenses. (see Rental Properties in the Information Section)
- Other Income: Please include details of any other income you have received during the year.

- Work Related Motor Vehicle Expenses: See Travel Expenses in the Information Section
- Other Work Related Travel Expenses: See Travel Expenses in the Information Section
- Work Related Uniforms/ Protective Clothing/ Laundry Expenses: Including purchase of uniforms, protective clothing, shoes, laundry & dry cleaning
- Work Related Self Education Expenses: See Education in the Information Section

- S & A Insurance
- Stationery
- Subscriptions
- Tool Replacement Receipts
- Work Related Telephone/Mobile/Internet use
- Interest and Dividend deductions: (including bank fees & taxes, investment advice)
- Gifts or Donations to Eligible Organisations: See Donations in the Information Section
- Cost of Managing Tax Affairs: Your Tax Agents Fees from the previous year.
Other Information Required:
- Do you have Private Health Cover? (include health fund statement)
- Do you have a spouse? Please provide your spouse’s name, taxable income & any Centrelink Benefits received
- Did you make any personal contributions to a superannuation fund? Please provide details
- Did your medical expenses for the year total more than $1,500? If so, please provide receipts. See Medical Expenses in the Information Section for details on what medical expenses you can claim.
